Forms

The following are commonly needed forms relating to your child's evaluation.

1) Permission Form to test your child
OR Adult Permission Form to test (if person to be tested is 18 or older)

2) Parent Questionnaire
OR Re-evaluation Parent Questionnaire (if your child was previously seen by Dr. Hill)

3) Teacher Input Form (Should be filled out by your child's primary/home room teacher. For middle school and high school, this form should be filled out by two of the child's core teachers.)
OR Preschool/Kindergarten Teacher Input Form (If your child is in preschool or kindergarten, please have your child's teacher fill out this instead.)

5) For the comprehensive or neuropsych evaluation, there are additional forms required for the parent and teacher. Please contact Dr. Hill directly for those forms.

Please note: If you would like Dr. Hill to share the results of your child's testing with another physician, an educator, the school or anyone else who is not the child's legal guardian, by law, the parents or legal guardian must complete the Permission to release test results below.
